This year there are 2 parts to the Health Risk Assessment: The HRA questionnaire – Complete by June 3 to ensure that cost sharing on your insurance is waived Biometric Screening – OPTIONAL but encouraged! Biometric screening clinics will be held at the Wellness Clinic. Come out and learn your numbers: cholesterol, diabetes, BP and weight Meet with Wellness Clinic Staff Receive an insulated lunch
