Health Risk Assessment

**HRA Clinics are postponed**

UNC Family Medicine and Town Staff have made the decision to temporarily postpone the in-person HRA Drive, based on general recommendations from UNC Healthcare, Center for Disease Control, and North Carolina Department of Health & Human Services. This will lower any potential risk of COVID transmission in any Wellness Clinic programs where larger groups could convene. In addition, Wellness Clinic staff have increased demands right now addressing employee questions, concerns and minor illness.

We will still go ahead with the HRA questionnaire; you can complete the questionnaire on our secure online portal at http://www.chapelhillwellnessatwork.org?  After you log in, click on “Click here to start the HRA”. If you don’t know how to log in, contact the clinic for assistance (919-968-2796). For those that need a paper copy, we will make that available too.

For any questions about COVID, UNC Health has a hotline (919-445-5000). UNC’s web site on coronavirus is at https://www.unc.edu/coronavirus/. The NC Department of Health COVID web site is at https://www.ncdhhs.gov/divisions/public-health/coronavirus-disease-2019-covid-19-response-north-carolina.

Thank you,

Wellness Clinic Staff

Participating in the annual Health Risk Assessment (HRA), held every spring, helps to keep employees healthier and reduces healthcare costs, by identifying potential health concerns early.  Employees who are enrolled in the Town’s health insurance program are eligible to participate.  Take the HRA every year to be aware of potential health problems and be eligible for incentives.

In the Annual HRA, you will be asked to:
  1. Complete an on-line questionnaire to assess your health behaviors and lifestyle.
  2. With a finger-stick, give a small blood sample for diabetes and cholesterol screening.
  3. Get your weight and blood pressure checked.
In return you will receive:
  • A personalized report of your risk for disease and health coaching to improve your overall wellness
  • Cost-sharing contribution for health insurance is waived
  • Satisfaction knowing you are supporting the Wellness@Work program
  • Insulated lunch bag with Wellness@Work logo
HRA Incentives:

Set a personal health goal (e.g. weight loss or lower blood pressure) and receive a $100 gift card if you meet your goal in six months!  Maintain your goal for another six months and receive another $100 gift card.

Get free access to Chapel Hill Parks and Recreation gyms and pools during open swim, lap swim, and open gym times.  When you complete your HRA, you will receive a facility access card that is good for one year (show the card with a photo ID for admission). (Employees who have not completed the HRA will be charged the resident or non-resident daily rate)

Permanent employees who have completed the HRA can join O2 Fitness for $5 per month.  Family members over the age of 12, partners, part-time employees and employees who have not completed the HRA (including all employees who are eligible to join BCBSNC) are eligible to join O2 Fitness for $10 per month.  Contact Stacy Ramos at sramos@townofchapelhill.org or 919-969-5029 to sign up.

If it is unreasonably difficult due to a medical condition for you to achieve the standards for the reward under this program, or if it is medically inadvisable for you to attempt to achieve the standards for the reward under this program, please call 919-968-2796, and we will work with you to develop another way to qualify for the reward.

Frequently Asked Questions

What happened to the $25 gift cards for completing the HRA?

The gift cards can no longer be given out. The grant that funded the gift cards has ended, and moving forward, employees would have to report such gift cards on their taxes.

What will I receive for participating in the HRA?

  • Information to improve your health
  • Cost-sharing contribution for health insurance is waived
  • A personalized plan for follow-up based on your health needs and goals
  • Insulated lunch bag with Wellness@Work logo

Does participating in the HRA mean that my cost-sharing contribution for health insurance is waived?

Yes, all employees who participate in the HRA will have their cost-sharing contribution for health insurance waived for that year. To keep the contribution waiver from year to year, the employee must participate every year.

Do I need to do the HRA every year? Do I need to complete the questionnaire every year?

The HRA Drive is held every spring and all employees enrolled in the Town’s Health insurance program are to participate every year, including completing a new questionnaire. By participating every year, you can be aware of potential health problems and set health goals with Wellness Clinic Staff. Also, by participating every year, your contribution for health insurance will be waived every year.

Can I use lab results done at my doctor’s?

Yes, if the results were within the past 6 months we can use them for the HRA. Please bring the results with you to the HRA clinic. The results we need are: Hemoglobin A1C, Total Cholesterol, HDL Cholesterol and LDL Cholesterol.

What are the incentives for reaching personal health goals?

Employees can set a goal for Tobacco cessation, 10% weight loss, improved blood pressure, diabetes, and improved cholesterol.  If you meet that goal in 6 months, you will receive a $100 gift card.

Maintain your goal for another 6 months and receive another $100 gift card.

Do I have to make an appointment for the HRA or can I “walk-in”?

During a HRA clinic, staff are able to see up to 40 employees. Making an appointment allows staff to better prepare but walk-ins are ALWAYS welcome (up to 40 participants per clinic).

I can’t make it to any of the HRA Clinics. Are there other times I can do it?

Yes, you can come to any of the Wellness Clinic’s usual Walk-In Hours unless we are off site conducting a HRA Clinic. You can also schedule a routine clinic appointment.

I tried to log in to the Portal to schedule my HRA and to do my health questionnaire but couldn’t get in. What should I do?

If you have any trouble logging in to the portal or have forgotten your log in and password, please call the clinic and staff will assist you. The clinic number is 919-968-2796.
