Health Risk Assessment

The 2023 Health Risk Assessment Drive has begun! All you need to do is complete the HRA questionnaire. The in-person biometric screening is optional (but encouraged) this year. Complete the HRA by June 3 to ensure that cost sharing on your health insurance is waived.

You can complete the questionnaire online or on paper. If you are signed up to UNC My Chart, you will receive a message with the questionnaire attached. If you don’t want to do the questionnaire through UNC My Chart, you can fill out a paper copy and it will be entered on your behalf.

If you aren’t signed up for UNC My Chart, go to MyUNCChart.org to sign up for an account.

Biometric Screening Clinics will be held at the Wellness Clinic in the lower level of the Public Housing office at 317 Caldwell St. Ext. You don’t need to make an appointment during clinic times.

  • Learn your numbers—cholesterol, diabetes, blood pressure, and weight
  • Meet with Wellness Clinic Staff
  • You’ll get a prize (insulated water bottle or lunch bag)

Participating in the annual Health Risk Assessment (HRA), held every spring, helps to keep employees healthier and reduces healthcare costs by identifying potential health concerns early.  Employees who are enrolled in the Town’s health insurance program are eligible to participate.  Take the HRA every year to be aware of potential health problems and be eligible for incentives.

In the Annual HRA, you will be asked to:

  • Complete an on-line questionnaire to assess your health behaviors and lifestyle.

In return you will receive:

  • A personalized report of your risk for disease and health coaching to improve your overall wellness
  • Cost-sharing contribution for health insurance is waived
  • Satisfaction knowing you are supporting the Wellness@Work program

HRA Incentives:

Set a personal health goal (e.g. weight loss or lower blood pressure) and receive a $100 gift card if you meet your goal in six months!  Maintain your goal for another six months and receive another $100 gift card.

Get free access to Chapel Hill Parks and Recreation gyms and pools during open swim, lap swim, and open gym times.  When you complete your HRA, you will receive a facility access card that is good for one year (show the card with a photo ID for admission). (Employees who have not completed the HRA will be charged the resident or non-resident daily rate)

Permanent employees who have completed the HRA can join O2 Fitness for $5 per month.  Family members over the age of 12, partners, part-time employees and employees who have not completed the HRA (including all employees who are eligible to join BCBSNC) are eligible to join O2 Fitness for $10 per month.  Contact Tom Clark at tclark2@townofchapelhill.org or 919-969-5031 to sign up.

Frequently Asked Questions

What happened to the $25 gift cards for completing the HRA?

The gift cards can no longer be given out. The grant that funded the gift cards has ended, and moving forward, employees would have to report such gift cards on their taxes.

What will I receive for participating in the HRA?

  • Cost-sharing contribution for health insurance is waived
  • A personalized plan for follow-up based on your health needs and goals

Does participating in the HRA mean that my cost-sharing contribution for health insurance is waived?

Yes, all employees who participate in the HRA will have their cost-sharing contribution for health insurance waived for that year. To keep the contribution waiver from year to year, the employee must participate every year.

Do I need to do the HRA every year? Do I need to complete the questionnaire every year?

The HRA Drive is held every spring and all employees enrolled in the Town’s Health insurance program are to participate every year, including completing a new questionnaire. By participating every year, you can be aware of potential health problems and set health goals with Wellness Clinic Staff. Also, by participating every year, your contribution for health insurance will be waived every year.

What are the incentives for reaching personal health goals?

Employees can set a goal for tobacco cessation, 10% weight loss, improved blood pressure, diabetes, and improved cholesterol.  If you meet that goal in 6 months, you will receive a $100 gift card.

Maintain your goal for another 6 months and receive another $100 gift card.
